 To maintain excellent limb function after tumor resection in patients with soft-tissue sarcomas, we developed and established a new therapeutic modality that consists of acridine orange excitation with photons and irradiation after intra-lesional or marginal resection that preserves the normal tissue of major nerves, vessels, or bones adjacent to the tumor. Our acridine orange therapy procedure was used as part of photodynamic surgery, photodynamic therapy, and radiodynamic therapy. In this study, we reviewed 26 cases of primary soft-tissue sarcoma treated by surgery combined with acridine orange therapy. The results showed a low local recurrence rate (7.7%) and high local recurrence-free rate (88%) after acridine orange therapy. The limb function of all patients was maintained at 100% according to Musculoskeletal Tumor Society limb function criteria. Based on these results, we concluded that acridine orange therapy is useful for local control and maintenance of excellent limb function after margin-positive tumor resection.
