神経障害性脊椎症に対する手術的治療

Surgical Treatment of Charcot Spine

　神経障害性脊椎症は痛覚や固有知覚の欠如により，繰り返される外力によって脊椎に高度の不安定性と破壊が生じる疾患である．われわれは本疾患の6例に対して手術的治療を行った．手術においては，前方より病巣掻爬と自家骨移植を行い，後方より強固なinstrumentationによって固定を行うのが原則である．骨破壊が軽度であれば，後方椎体間固定術によっても対処できる．また，神経障害性脊椎症に感染を伴った2例を経験した．不安定性が強く，感染を伴っている症例には，脊椎創外固定が有用であった．

　Neuropathic spinal arthropathy is a relatively rare condition, but must be considered in the differential diagnosis of degeneration of one or more levels of the spine associated with diminished protective sensation. This is a report of 6 cases of Charcot spine treated surgically. The main therapeutic modality of the surgical treatment was combined anterior and posterior approach, with extensive debridement, autogenous bone grafting, and posterior instrumentation. Some cases with mild bone destruction could be treated by posterior interbody fusion. There were two rare cases of Charcot spine with infection, and external fixation is a useful method of treating such patients.
