We presented a case of failure of prophylactic antibi-otic against infectious endocarditis (IE). A 54-year-old man with a history of mitral valve prolapse and regur-gitation was given ofloxacin as a prophylaxis after dental extraction and developed IE. He had no clinical evidence of malignancy or immunodeficiency. Blood cultures grew α haemolytic streptococcus (S. sanguis). The organism was resistant to ofloxacin and susceptible to benzylpenicillin. He was successfully treated with benzylpenicillin for the initial 3 weeks but mitral valve replacement was carried out because of systemic embo-lism. The American heart association recommended for prophylaxis of endocarditis 3 g of amoxycillin by mouth one hour before dental extraction. Viridans streptoccoci are frequently resistant to newquinolone in Japan. It seems that these agents are unsuitable for such prophylaxis.
