Usefulness of amyl nitrite inhalation test in evaluation of coronary artery disease:clinical and experimental studies

背景

　1867年にBruntonが狭心痛の治療薬として亜硝酸アミル（以下AN）を使用して以来，100年が経過した^1）。しかし現在は，その臭気や副作用に加えて取り扱いの不便さから，狭心発作の治療薬の座はニトログリセリンにとってかわられ，ANはもっぱら負荷心音図法の一手段として心雑音の鑑別に使用されている。

　一方Controらは，冠動脈疾患（CAD）例にANを吸入させると，約半数において心電図ST・TがMasterの運動負荷によると同様の虚血性変化を示したと述べ，この作用を"paradoxic action（奇異性反応）"と呼んでいる^2）。

The effect of amyl nitrite inhalation (ANI) was investigated if this maneuver could be used as a stress test for evaluation of presence and severity of coronary artery disease (CAD).

On clinical studies, systolic time intervals were measured in 68 patients (46 with CAD and 22 with no-CAD) before and after ANI. While increase in ET/PEP after ANI was significant in no-CAD group, it was not in CAD group,and there were statistically significant differ-ences (p<0.001) between the two groups.
