The vectorcardiogram of the ventricular septal defect with severe pulmonary hyper-tension was examined. On the basis of total pulmonary vascular resistance (TPVR), all 42 cases were divided into three groups. The group 1 includes those with TPVR over 1,001 dynes・sec/cm5, the group 2 includes those with TPVR between 421-1,000 dynes・sec/cm5, and the group 3 includes those with TPVR less than 420 dynes・sec/cm5. The initial vector was directed to the right anterior more frequent in the group 2 than others. The maximum vector was moved from the left posterior to the left anterior or right anterior, according to the increase of TPVR. The terminal vector was located far more right posteriorly in the group 1 and group 3 on both the frontal and hori-zontal projections, and posteriorly superiorly in the group 1 at the right saggital plane. The clockwise inscription in the horizontal plane of the QRS loop was recognized frequ-ently in the group 1, occasionally in the group 2, and never in the group 3.
