原発巣と転移巣の病理組織像が明らかに異なった胸腺腫の1例

Two Different Pathohistological Findings between the Primary and Metastatic Region in Thymoma

要旨　64歳の女性で，原発巣と転移巣の病理組織所見が異なった浸潤型胸腺腫の稀な1例を経験した．原発巣である前縦隔腫瘍に対して行った胸腔鏡下生検で，浸潤型胸腺腫(Type B1，臨床病期Ⅳa)の初期診断を得たため，多剤併用化学療法を推奨する説明を行った．しかし，同意が得られず，経過観察となった．1年後，転移性脳腫瘍が出現し，腫瘍摘出を行った．転移巣の病理組織所見は，原発巣の所見と異なり，胸腺癌であった．多剤併用療法が選択され治療を行ったが，軽度の縮小傾向しか認めなかった．時間的経過および空間的変化によって，原発巣と転移巣が異なる病理像を呈していると考えられたが，その詳細は不明である．

　A rare case showing that different pathohistological findings were obtained between the primary and the metastatic region in a 64-year-old female. Early diagnosis was primary invasive thymoma, type B1, clinical stage Ⅳa obtained by surgical mediastinoscopic biopy in an anterior mediastinal tumor. Pathohistological findings of thymic cancer were found in metastatic brain tumor about 1 year after management with the best supportive care. Four cycles of combination chemotherapy with doxorubicin, cisplatine, vincristine and cyclophosphamide(ADOC therapy)evoked a slight response, but the outcome was invasive thymoma, type B1.
