Summary

 Plasma endothelin(ET)-1 levels were reported to increase concordantly with the severity of heart failure. Using neurohumoral factors including ET-1, echocardiography and MIBG myocardial imaging, we investigated the usefulness of angiotensinⅡreceptor blockers(ARB)in patients with chronic heart failure(CHF). ARB(losartan or candesartan)was also administered to 31 patients with CHF. Both before and 6 months after administration, neurohumoral factors (ANP, BNP, aldosterone, plasma renin activity, norepinephrine and ET-1) were investigated, and echocardiography and MIBG(in 25 patients)were performed. ANP and BNP levels significantly decreased after the ARB administration. ET-1 levels significantly decreased in both patients pretreated with(n=20) and without(n=11) angiotensin converting enzyme inhibitors. There were no significant changes in the heart-to-mediastinum ratio shown on MIBG, but the washout rate significantly decreased after ARB administration. Changes in the ET-1 level correlated positively with changes in the BNP level and the MIBG washout rate. Thus, the suppression of the endothelin system may play some role in the efficacy of ARB on heart failure.
