 This article describes the present and future state of a gastrography-based gastric cancer screening program. We retrospectively examined the gastrographic images and clinicopathological findings of 81 gastric cancers detected by our cancer screening program. The results indicate that gastrographic images can identify gastric cancers which are more than 2cm in size or invade deeper than the submucosal layer. Moreover, the ratio of gastric cancers identified at an early stage in the annually screened group was relatively higher than that of the initially screened group. Thus, we conclude that the annual gastrography-based gastric cancer screening program is effective in decreasing gastric cancer mortality. However, it has been difficult to maintain the effectiveness of the cancer screening program, because of the decrease in gastric cancer morbidity and mortality among the younger generation of Japanese people, who are in their 40s. We propose that Helicobacter pylori infection should be used to define a high-risk individuals among the young population to perform an effective cancer screening program.
