 We studied the diagnostic value of magnifying endoscopy with narrow band imaging for detection of specialized intestinal metaplasia(SIM)in Barrett's mucosa/esophagus. Findings of magnifying endoscopy using narrow band imaging were prospectively compared with histological findings. The most characteristic patterns of SIM were revealed to be cerebriform-like fine mucosal pattern(FMP)and ivy or DNA-spiral-like capillary pattern(CP). The addition of CP to FMP improved accuracy in diagnosing SIM(p<0.0001). These results suggest that observation by magnifying endoscopy with narrow band imaging may improve diagnostic ability for detecting SIM.
