Endoscopic Aspiration Mucosectomy

要旨　内視鏡的吸引粘膜切除法（EAM）の基本手技とコツにつき詳述した．EAM法は，2チャンネル法の手技上の弱点を克服すべく開発した手法で，吸引により病変を挙上するため，病変存在部位によらない確実な切除が可能で，スネアを先端フードの外周にセットするため，吸引各法の中では，より大きな切除が可能であるという特長を有する．実際の手技施行にあたっては，手技の各ステップに，開発意図に沿った正しい方法があり，アスピレーション・ムコゼクター，局注針，局注液など，デバイス類の選択とその基本的なセッティングが極めて重要であり，内視鏡操作技術とともに，これらが手技の成否を最終的に左右すると言っても過言ではない．

　The basic procedure for endoscopic aspiration mucosectomy was described.

　EAM was developed for solving the weak points of the two-channel method for endoscopic mucosal resection. Because the lesion is lifted by suction in this method, it can be used to resect tumors anywhere in the stomach. The EAM can get larger specimens than other suction methods because it is equipped with a snare wire outside the suction hood of the device, called an aspiration mucosector.

　There is an appropriate procedure for each step in the EAM. Choice and preparation of the devices are as indispensable as the endoscopy technique itself.
