要旨　当院では，2000年4月より切開・剥離法（endoscopic submucosal dissection; ESD）を導入し，さらに，2003年4月からは術前検査として全症例に対し狭帯域フィルター内視鏡（narrow band imaging; NBI）システムを併用した拡大内視鏡観察を施行している．その結果，通常観察で進展範囲診断が困難な粘膜表層を進展する病変であっても，拡大観察とNBIシステムを併用し粘膜微細構造の変化や異常毛細血管の増生を観察することで正確な切除範囲決定を行える可能性が示唆された．今回は，当院におけるNBIシステム導入前後のESDの治療成績を検討するとともに，特に本システムが有用と考えられた2症例を提示する．

　We have been conducting the endoscopic submucosal dissection method (ESD) for resection of adenoma and early stage gastric cancer since April, 2000. Additionally, magnifying endoscopy combined with the narrow band imaging (NBI) system was introduced as a preoperative examination to evaluate lateral spread of the lesions from April, 2003. Magnifying endoscopy combined with the NBI system allowed clear visualization of fine mucosal patterns and capillary structure and gave precise definition of lesion boundaries that was impossible with conventional endoscopy. In this study, comparison was made of outcomes of ESD with and without evaluation of the lateral spread of the lesions using magnifying endoscopy combined with the NBI system.
