 [Aim]To compare the short-term outcomes of laparoscopic distal pancreatectomy(LDP) and robot-assisted distal pancreatectomy(RDP). [Methods]The study included 115 patients who underwent distal pancreatectomy performed at our department from April 2012 to March 2023. [Results]Following the introduction of a spleen-preserving method, 36 cases of RDP and 52 cases of LDP were performed. Underlying disease and tumor factors did not differ between the two groups. On the other hand, the rate of successful spleen-preserving DP was 75.8%in the LDP group and 95.0%in the RDP group, showing a trend toward higher rate in the RDP group(p=0.08). The operative time was significantly longer in the RDP group(p<0.01), but there was no difference in other short-term outcomes. No difference was observed in short-term outcomes between the L-SPDP and R-SPDP groups except for the operative time. [Conclusions]Robotic-assisted surgery appears to be a useful approach for spleen preserving distal pancreatectomy.
