認知機能低下を有する整形外科疾患患者に関する回復期リハビリテーション病棟退院時申し送り票の作成

Development of a new handover sheet for patients with orthopedic disorders and cognitive impairment discharged from a convalescent rehabilitation ward

要旨：認知機能が低下した整形外科疾患患者に関する退院時の申し送りについて，療法士と介護スタッフが重視する情報項目を明らかにし，新たな申し送り票を作成することを目的とした．療法士25名，介護スタッフ45名を対象にしたアンケートの結果，ADLや転倒リスク，認知機能に加え，性格や生活習慣等の心理社会的情報が双方にとって重要であることが明らかになった．認知機能の低下を有する患者が病院から地域へと環境が変化する際に，療法士と介護スタッフが連携強化を図ることで，個別的支援の質向上に寄与すると考えられる．一方で，対象地域および機関が限定的であり，対象者自身の要素が不足するなどの課題も示唆された．

　This study aimed to develop a new handover sheet for patients with orthopedic disorders and cognitive impairment, by identifying the information items that therapists and care staff consider important. The results of a questionnaire administered to 25 therapists and 45 care staff revealed that in addition to ADL, fall risk, and cognitive function, psychosocial factors, such as personality and lifestyle were important to both groups. When patients with cognitive decline move from the hospital to the community, strengthening collaboration between therapists and care staff may contribute to improving the quality of individualized support. Conversely, the target areas and institutions were limited, and issues such as the lack of a client's own factors were also suggested.
