はじめに
血圧は循環の総合指標であり,臨床現場で最も広く用いられる生体情報の一つである。心拍出量と血管抵抗の相互作用として規定される血圧は,全身臓器の灌流と酸素需給を反映するため,日常診療から集中治療まで幅広い場面で重視されてきた。特に周術期は麻酔薬や手術侵襲により自律神経反応や体液動態が大きく変化し,それが血圧に反映される。極端な血圧変動は臓器虚血を招き,重篤な合併症や予後悪化につながるため,適切な管理が不可欠である。本論文では,血圧の成り立ちを整理し,その生理学的意味を心力学的視点から概説する。これらの基礎的理解は,周術期血圧管理を考えるうえで重要な出発点となる。
Blood pressure(BP), an integrative indicator of the body’s circulation, is determined based on the product of cardiac output and systemic vascular resistance. Blood pressure serves as the driving force for organ perfusion and oxygen delivery, making its regulation essential across a wide range of clinical settings. The regulation of BP relies on hierarchical mechanisms in which the central arterial pressure is stabilized while the local vascular tone adjusts the regional blood flow. In addition, BP is regulated by various modulatory factors, including short-term control via the baroreflex, neurohumoral regulation, and the pressure natriuresis relationship. Importantly, the optimal BP is not uniform;it depends on organ-specific perfusion thresholds and the presence of comorbidities. This article summarizes the physical and hemodynamic basis of BP, its relation to ventricular-arterial coupling, and myocardial energetics, and it provides a physiological perspective of BP regulation for subsequent discussions on perioperative management.
