重篤な機能障害を呈する高齢者との協業による自伝作りの効果

Effects of a collaborative biography on an elderly depressed patient

要旨：介護老人福祉施設に入所中の重篤な身体障害を呈する高齢者に対し，作業療法士と協業して「自伝を作る」という作業活動を導入した．自伝作りにおいて対象者は主に過去を自由に語り，「自伝を作る」という作業は対象者に確認を取りながら作業療法士が行った．自伝作りの介入前後で，自己評価式抑うつ尺度のスコアは低下し，改訂版PGCモラールスケールのスコアは上昇した．また，完成後に職員が作成した自伝を読むことで，対象者へ対する認識やケアのあり方が変化した．今回の「自伝作り」には，抑鬱的であった対象者が自尊心を回復する，対象者のQOLが向上する等という効果のあることが示唆された．

　In a collaborative effort, we wrote the biography of an elderly patient suffering from acute depression at a special nursing home for the elderly. She freely narrated stories about her past to her occupational therapist, who then constructed her biography based on these stories. This intervention showed positive results. Her score on the self-rating depression scale (SDS) decreased while that of the Philadelphia Geriatric Center Morale Scale (PGC) increased. Upon reading her biography, the care staff changed their methods of care and manner of contact. The construction of biographies may be useful for clients of depression in instilling pride, and possibly improving QOL.
