マラリア患者が意識障害を呈した場合は脳マラリアを考える。マラリア原虫は赤血球に感染するが，神経細胞に感染しない。脳マラリアは，マラリア原虫が感染した赤血球が脳の微小血管の血管内皮細胞に接着し，血管内腔の狭小化や閉塞をきたすことが発症の端緒になると考えられているが，詳細な病態は解明されていない。脳マラリアを疑った場合は，可及的すみやかに抗マラリア薬であるアーテスネート製剤の静注を行う。

Abstract

When patients with malaria exhibit impaired consciousness, cerebral malaria should be considered. Although malaria parasites infect red blood cells, they do not infect neurons. Cerebral malaria is thought to occur when parasite-infected red blood cells adhere to endothelial cells lining the brain microvasculature, obstructing the vascular lumen; however, its detailed pathophysiology remains unclear. If cerebral malaria is suspected, intravenous artesunate should be initiated as soon as possible.
