延髄に伸展した頸髄上衣腫の手術

Microsurgery of Cervical Intramedullary Ependymomas Extending into the Medulla Oblongata

Ⅰ．はじめに

　脊髄上衣腫は最も頻度の多い髄内腫瘍であるが，境界が明瞭で重大な後遺症を残すことなしに全摘出が可能であると考えられている^2,4,5,8）．さらに，多くの症例で脊髄上衣腫の全摘出後に一時的に症状が悪化するが，数カ月後には回復し，長期的に追跡すると術前より症状が改善することが報告されている^12,13）．また，組織型が良性であることがほとんどであり，全摘出を行った場合，放射線治療などの補助療法を施行しないでも再発の可能性は非常に少ないというのがコンセンサスとなっている．これらのことより，脊髄上衣腫に対しては，全摘出が治療の第1選択として推奨されている．特に，頸髄に存在する上衣腫は胸髄や腰髄膨大部に存在するものに比較し手術後の予後が良いことが報告されている^8）．しかしながら，頸髄上衣腫の中でも，上位頸髄に存在し延髄に伸展する上衣腫の全摘出には躊躇を覚える．というのは，延髄には呼吸中枢や循環中枢など，生命の維持に必要な中枢が存在し，手術侵襲によりこれらの中枢に影響が及ぶことを危惧するからである．今回，延髄に伸展した頸髄上衣腫の3例を報告し，それらの解剖学的特徴を明らかにするとともに，全摘出の方法を記述したい．

Intramedullary spinal cord ependymomas are well-demarcated tumors, which can be totally resected without significant morbidity. However, it is challenging to remove intramedullary ependymomas extend-ing into the medulla oblongata, because the medulla oblongata has such important roles as regulation of re-spiration, circulation, and digestion. We describe here three cases of intramedullary ependymomas extend-ing into the medulla oblongata. The tumors were localized in the dorsal part of the caudal medulla oblon-gata and were surrounded by the nuclei and tracts related to the dorsal columns. The tumors were safely exposed by dividing the medulla oblongata along the posterior median sulcus and resected from the sur-rounding normal tissues. Although patients demonstrated transient dorsal column dysfunction postoper-atively, the symptom finally recovered.
