選択的腎動脈線溶療法を行った腎梗塞

A CASE REPORT OF RENAL INFARCTION TREATED WITH PERCUTANEOUS TRANSLUMINAL RENAL ARTERY RECANALIZATION

　39歳，男性．左側腹部痛を主訴に来院，背部叩打痛の持続，LDHの異常高値と造影CTより腎梗塞と診断した．発症72時間後，血栓を認めた腎動脈背側枝内ヘウロキナーゼを選択的に注入する線溶療法を試みたところ，自覚症状，LDH値の改善を認めた．2ヵ月後の造影CTにおいて梗塞領域の造影効果がみられ，選択的腎動脈線溶療法は腎梗塞に対し有用な治療法と考えられた．

A 39 year-old man with an abrupt onset of severe left flank pain showed emboli of segment arteries inthe left kidney demonstrated by computed tomography and angiography.72 hours after the onset, he wastreated with selective intraarterial infusion of urokinase. Repeated angiography showed resolution of emboliand recanalization of renal arteries. Two months later, recovery of left renal function was determined bycomputed tomography. We concluded that percutaneous transluminal artery recanalization might be a usefultherapy for renal infarction.
