不安定型骨折に対する髄内釘固定において，安定型整復位を獲得できない可能性や，後外側骨片や髄内釘のswing motion，lag screwの偏心性挿入による術後の整復位破綻の可能性は低くない．骨粗鬆症による骨梁構造の変性から強固な把持力が発揮できず破綻する可能性もある．従来型のsingle lag screwの髄内釘での対応は困難であり，回旋安定性や補助的な固定力を要するインプラントの選択や，骨頭の海綿骨とlag screwのinterfaceへの骨補填材追加も考慮する必要がある．
