 Fondaparinux sodium (FPX) has been demonstrated to be of value in preventing venous thromboembolism (VTE) after joint replacement surgery. We report two cases of acute pulmonary embolism following deep venous thrombosis after total hip or unicompartmental knee arthroplasty in which FPX had been used to prevent VTE. We speculated that VTE had occurred in both cases after the period when FPX was effective. We discuss the impossibility of completely preventing VTE in patients treated with anticoagulants. Further study of the prevention of VTE is needed in regard to the optimal period for prevention and methods of prevention.
