非定型大腸Crohn病―手術例における肉眼的・組織学的検討

Atypical Cases of Crohn's Disease: Macroscopic and Histologic Characteristics of the Surgically Resected Cases

要旨　大腸Crohn病で臨床的に非典型例とされた手術例5例を取り上げ，典型例と肉眼的・組織学的に比較検討した．潰瘍性大腸炎と鑑別が困難であった2例は，全結腸に連続性の病変がみられた．肉眼的には，縦走する潰瘍はみられたが，数が多く，長径が短く，幅が狭く，不整形の潰瘍も多く，典型的なCrohn病にみられる縦走潰瘍とは異なっていた．病変部のcobblestone像も典型的ではなく，非潰瘍性粘膜の炎症が強かった．小さな潰瘍が多発し，周辺粘膜および粘膜下層に炎症を及ぼした結果，Crohn病の特徴である区域性のある線としての病変が明瞭に現れておらず，むしろ面としても病変として認識されるような病変になったために，潰瘍性大腸炎との鑑別が困難になったものと思われた．結核との鑑別を要した3症例は，非連続性の病変で，潰瘍は縦走傾向を示すが長径は短く，幅が広く，また潰瘍瘢痕，横走する潰瘍がみられるなど，典型的なCrohn病にみられる縦走潰瘍とは異なっており，cobblestone像も典型的ではなかった．Crohn病の経過で典型的でない潰瘍がみられた症例で，潰瘍がほぼ同一の時期にあり，瘢痕帯がない点，回盲部の変形がない点などから結核と区別できる病変と思われた．

　To evaluate characteristic features of atypical case of Crohn's disease, five patients were analyzed. Two of them were confused with ulcerative colitis (case 1 and 2) and 3 cases mimicked tuberculosis (case 3, 4 and 5).

　The characteristic features of the cases confused with ulcerative colitis were as follows: 1) continuous lesion of the entire colon, 2) multiple short narrow longitudinal ulcers and irregular-shaped ulcers, compared with typical cases, 3) no classical cobblestone appearance.

　On the other hand, the characteristic features of the cases mimicking tuberculosis were as follows: 1) segmental involvement of the entire colon, 2) a few short broad longitudinal ulcers and transverse ulcers, 3) no classical cobblestone appearance.
