 The patient was a 70-year-old woman, who presented microcytic hypochromic anemia and was referred to our hospital for further examination of the stomach. Upper gastrointestinal series and endoscopy showed a flat elevated lesion, approximately 3 cm in diameter, in the anterior wall of the upper gastric body. An uncommon aggregation of white and red speckles was also seen on the mucosa around the elevated lesion. A biopsy specimen of the elevated lesion showed papillary adenocarcinoma, so total gastrectomy was performed. Macroscopic examination of the resected tissue showed that the elevated lesion, measuring 36×29 mm in size, was surrounded by an aggregate of white and red speckles extending on the mucosa. Histologically, the elevated lesion was papillary adenocarcinoma invading the deep layer of the submucosa and focally involving the superficial propria muscularis, as well as being accompanied by prominent lymphangiosis carcinomatosa. Lymphatic vessels around the elevated tumor showed marked dilatation because of tumor thrombosis and hemorrhage especially in the propria mucosae, thus presenting such an uncommon aggregation of speckles.
