Endoscopic Dilatation for Ileo-colonic Strictures

要旨　下部消化管狭窄，特に良性狭窄には，主としてバルーンを用いた内視鏡的拡張術が施行されている．本法は偶発症なく完遂し有効な場合は侵襲が少なく利点の多い手技であるが，穿孔が発生すれば緊急手術の回避が困難であることを留意しなければならない．術前に内視鏡およびEUSで狭窄部の線維化の程度を判定して，穿孔を防止し有効な拡張が得られるための最適な方法を検討し選択する必要がある．拡張術の施行は，緊急手術を含めた外科的治療が可能な全身状態にあることが前提条件である．内視鏡的拡張術は外科的治療と対比して十分な理解を得たうえで選択されなければならない．

　Endoscopic balloon dilatation is now accepted as a useful therapy for stenosis with inflammatory lesions and anastomotic strictures in the small and large intestine. However, this method may lead to severe complications including perforation. Endoscopic therapy such as electroincision and surgical procedure might be a useful alternative option for the treatment of stricture. EUS examination before treatment is recommended for disclosing the pathological findings of the affected site and providing crucial clues regarding the indication for balloon dilatation. The most effective method should be selected for dilatation of the stricture under thoroughly informed consent.
