Abstract Purpose:Uveitis-glaucoma-hyphema(UGH)syndrome is a rare complication of cataract surgery which can cause uveitis, recurrent anterior chamber hemorrhage, and glaucoma. We report a case of UGH syndrome that was diagnosed by inserting an intraocular microscope(3CCD Imaging System FT-203F, FiberTech)through a corneal sideport incision during a vitrectomy for an anterior chamber and vitreous hemorrhage of unknown origin.

Case:In May 2022, the patient presented with decreased visual acuity in her left eye(counting fingers at 20 cm), and anterior chamber and vitreous hemorrhages were observed upon examination. She was subsequently referred to the Department of Ophthalmology at Kagoshima University Hospital for further examination and treatment. She underwent vitrectomy the day after her first visit to our department. Using an intraocular microscope, we observed contact between the nasal haptics of the intraocular lens(IOL)and the ciliary body process, as well as blood clots accumulating in the same area. We diagnosed the patient with UGH syndrome and performed an IOL removal. Postoperatively, no recurrence of vitreous hemorrhage was observed, and an intrascleral fixation of the IOL was performed in September 2022.

Conclusion:UGH syndrome should be considered as a potential diagnosis in patients with unexplained uveitis, anterior chamber hemorrhage, and high intraocular pressure after cataract surgery. Intraoperative intraocular microscopy could be a useful diagnostic tool.
