  • Abstract

 After uninephrectomy (Nx) in healthy man, compensatory changes are adequate to maintain electrolyte and water homeostasis. The effector mechanism of this functional adaptation has been elucidated in experimental and human studies. In a previous study we examined glomerular and tubular function after Nx in living healthy renal transplant donors. It was found that the glomerular filtration rate (GFR) of the remaining kidney increased by 30% within the first 5 days after Nx. By use of the lithium clearance technique it wasdemonstrated that, initially, this increased filtrate load was passedfrom the proximal tubules to more distal nephron segments. Within2-4 weeks an increase was seen in proximal reabsorption of fluidleading to an almost complete normalization of glomerular-tubular balance.

 The factors responsible for the compensatory changes are un-known, but it has been suggested that circulating natriuretic sub-stances may be involved. The atrial natriuretic peptide(ANP) isassumed to participate in the maintenance of sodium and waterbalance. Thus, natriuretic action by direct renal effect, as well as amodulating effect on the renin-angiotensin-aldosterone system hasbeen ascribed to this hormone. To our knowledge no information isavailable about the involvement of ANP in functional adaptation toacute reduction in renal mass.
