肩関節拘縮に対する鏡視下授動術の術後成績―経時的変化

Arthroscopic Release for the Stiff Shoulder

　肩関節拘縮に対する鏡視下授動術の術後成績を経時的に検討した．肩関節拘縮に対して鏡視下授動術を行った21例24肩を対象とした．術前，術後1カ月，3カ月，6カ月，1年，および2年時に治療成績を評価した．日本整形外科学会肩関節治療成績判定基準(JOAスコア)の機能項目，VAS，および可動域は，術前と比較して術後1カ月という早期から改善が認められた．その後の経過は，はじめにJOAスコア機能項目と屈曲・外転角度，次にJOAスコアによる総合評価，夜間痛，および外旋・内旋角度が，最後に運動時痛の順に変化が認められなくなることが明らかになった．

　The clinical courses of 21 patients (24 shoulders) after arthroscopic release for stiff shoulder was reviewed.　The patients' clinical status was assessed before surgery and 1, 3, and 6 months and 1 and 2 years after surgery.　The JOA score for function, pain score on a VAS, and range of motion had improved at one month after arthroscopic release in comparison with the patients' preoperative status.　The results of subsequent follow-up of the patients' clinical courses revealed that improvement became unremarkable in firstly parameters as to JOA score for function, flexion, and abduction, secondly total JOA score, nocturnal pain, external rotation, and internal rotation, and lastly pain on motion.
