嚢胞性腫瘤の形態を示した膵神経内分泌腫瘍の1例

A case of pancreatic neuroendocrine tumor presenting as cystic tumor

膵神経内分泌腫瘍の多くは造影CTや造影MRIで境界明瞭な多血性腫瘍の像を示し，腫瘍径が大きいものでは嚢胞変性をきたすことが知られている。単房性嚢胞性腫瘤の像を呈し，粘液性嚢胞腫瘍と鑑別を要した膵神経内分泌腫瘍を経験したので報告する。

A 51-year-old female pointed out submucosal tumor on upper gastric body on upper gastrointestinal series in health checkup. There is no elevation of tumor markers. US showed the unilocular cyst with fluid-fluid level in the dorsal side of pancreas. CT demonstrated the cyst covered by smooth circumscribed margin with 34×36×37 mm in size. MRI revealed low intensity on T1WI, high intensity on T2WI, and no nodule or septum in the lesion. Laparoscopic distal pancreatectomy was performed and the diagnosis of pancreatic neuroendocrine tumor（G1）was made on histopathological examination. Pancreatic neuroendocrine tumor can present pure cystic tumor without peripheral enhancement.
