特発性椎骨動静脈瘻に対して経動脈的にコイル塞栓術を施行した1例

Endovascular treatment for spontaneous vertebral arteriovenous fistula：a case report

特発性椎骨動静脈瘻は，頭蓋外の椎骨動脈もしくはその分枝と周囲の静脈叢と直接短絡を形成する比較的まれな疾患である^1）。症状は，シャント量に依存し，無症候性で発見するものから拍動性耳鳴りや椎骨脳底血流不全に伴うめまい・脳虚血症状を呈す^2）ものまで多岐にわたり，まれに静脈圧上昇や機械的圧迫による脊髄・脊髄根症状を示すこともある^3）。今回我々は，耳鳴りの精査で発見された特発性椎骨動静脈瘻に対して経動脈的にコイル塞栓術を施行し，根治した症例を経験したので治療戦略を中心に文献学的考察を交えて報告する。

Spontaneous vertebral arteriovenous fistula（VADF）is an infrequent lesions characterized by abnormal communication of the extracranial vertebral artery or one of its branches to the surrounding venous plexuses. We describe a case report of a patient with VAVF presenting with tinnitus, which was successfully treated by coil embolization assisted with balloon for flow arrest. Point occlusion with embolization for VAVF without parent vessel occlusion can be a feasible, safe, and effective treatment.
