Purpose

The present study aimed to clarify the change for the efforts in the field of midwifery around the introduction of the Japan Obstetric Compensation System (JOCS) and to analyze important factors that contribute to enhancing obstetric care safety.

Subject and Method

Midwives in the Kanto region (N=97) were asked to fill in a self-rating questionnaire survey. There were 97 valid responses. The analysis was carried out using a one-way factorial ANOVA on the values before and after enforcement of the JOCS. We also extracted the multiple linear regression analysis with the enhancement of awareness in obstetric care safety as the dependent variable, and the background factors as the independent variable.

Results

Between the values of 40 items before and after, significant differences were seen in the positive change for efforts and management in pregnancy (5 items), during delivery (4 items), for the new-born (2 items) and general safety measures (11 items). No changes were seen in labor of in puerperium. The multiple regression analysis revealed that a higher number of years of clinical experience in midwives had corresponded to awareness of obstetric care safety.

Conclusion

The introduction of the compensation system influenced the various aspects of management in midwifery, and obstetric care safety awareness have been suggested as increasing with the clinical experience.
