糖尿病患者の食事の実行度と自己効力，治療満足度の縦断的研究

The Prospective Study to Make Clear the Relationship among Dietary Self-Efficacy, Hospitalization for Education, Treatment Satisfaction and Dietary Self-Management Behavior of Diabetes Mellitus

　本研究は，都内A病院に教育入院したNIDDM患者65名を対象に，教育入院と自己効力，治療満足度の関係，自己効力と治療満足度の関係，自己効力と食事の実行度の関係について把握することを目的に調査を行った．対象者のうち，退院時に食事療法のみの者32名については退院後6か月間縦断的に調査を行った．その結果から以下のことが示唆された．（1）教育入院により自己効力は有意に上昇した．入院時の患者の自己効力は，〈食事の自己評価〉，〈家族の協力〉と相関がみられた．（2）自己効力と治療満足度は相関がみられたことから，一方が上昇すれば他方も上昇するとことが示唆された．（3）退院時の自己効力は，退院後の〈食事の摂取率〉とはほとんど相関は認められなかったが，退院時の自己効力は，〈食事の自己評価〉と相関傾向がみられた．測定時期，食事の実行度の指標の検討が必要である．

　The purposes of this study are (1) to examine the change of self-efficacy and diabetes treatment satisfaction through experience hospitalization for self-management, (2) to make clear the relationship between dietary self-efficacy, treatment satisfaction and dietary self-management behavior in a 6 month period. Method: 65 patients with diabetes who enter hospital A in Tokyo for self-management training filled out a self-administered questionnaire. Then 32 of 65 patients who don't take medicine continued this study. Results: (1) Self-efficacy increased significantly through experiencing hospitalization for self-management, and was significantly related to their self evaluation of dietary and family support. (2) Self-efficacy was significantly related to diabetes treatment satisfaction, if self-efficacy increased, diabetes treatment satisfaction increased likewise. (3) Self-efficacy was significantly related to rating of diet on 10 levels, but it wasn't significantly related to self-management behavior. It is important to increase patients' self-efficacy and diabetes treatment satisfaction. We need to take into more consideration the scale of self-management behavior for dietary.
