上腰ヘルニアと鼠径ヘルニアを腹腔鏡下に同時修復した1例

A case of right superior lumbar and inguinal hernias repaired simultaneously by using laparoscopic technique

◆要旨：患者は76歳，男性．右腰背部膨隆を主訴に当科外来を受診した．腹部CTにて右上腰三角より上行結腸，脂肪組織の脱出を認め，右上腰ヘルニアと診断した．また，右鼠径ヘルニアも認めた．両ヘルニアを同一視野でアプローチするため，腹腔鏡下手術を選択した．手術台のローテーションを利用し体位変換することで4ポートでの同時修復が可能であった．術後経過は良好で，第3病日に退院した．上腰ヘルニアと鼠径ヘルニアを腹腔鏡下に同時修復した症例は検索しえた範囲内では初めての報告であり，手技は簡便で有用な術式であると考えられた．

A 76-year-old man visited our department with a chief complaint of a mass in the right lumbar area. Abdominal computed tomography revealed right superior lumbar and right inguinal hernias. To approach the hernias in the same view, we selected laparoscopic surgery. By rotating the surgical table, we successfully repaired both hernias via 4 ports. The patient was discharged from the hospital on the third postoperative day, without complications. This is the first report of right superior lumbar and inguinal hernias repaired simultaneously using laparoscopic surgery. We considered laparoscopic repair to be an effective technique because it is less invasive and simple.
