遊走脾に対する腹腔鏡下脾固定術

Laparoscopic Splenopexy for Wandering Spleen

　遊走脾は，脾固定靱帯の欠如により茎捻転やうっ血による脾腫を来す稀な疾患である．われわれは遊走脾の幼児例に対し，初めて腹腔鏡下脾固定術を行い報告したが，ここではその手技について述べた．脾の固定には脾を吸収性メッシュに包み，横隔膜下に縫着する方式を用いた，本症への腹腔鏡下手術によるアプローチは，脾の十分な観察ができ，捻転解除や摘脾への移行も可能であり，開腹術と同等の手術が安全に施行できる点から有用と考えられた．

Wandering spleen is an uncommon condition, showing hypermobility in the spleen, which causes torsion and infarction. This paper describes a technique of laparoscopic splenopexy for an infant with wandering spleen. Splenopexy was performed by anchoring the spleen wrapped in absorbable mesh. Laparoscopic approach and splenopexy seemed to be a feasible and efficacious procedure for management of wandering spleen.
