According to our clinical observations and theoretical speculations using a model of the middle ear, a large air cavity would serve to aleviatc the pressure changes due to Eustachian tube dysfunctions. Among varied methods of tympanoplasties, the methods to preserve the antrumcell cavities are most recommendable, because the restored large middle ear cavity is expected to facilitate postoperative recovery of the diseased middle car and to maintain the normalized middle ear cavity for good as well. The attic blockades, which arc common and frequent findings in chronic exudative, suppurative and cholesteatomatous otitis media, should be a special target of our attention in surgical and nonsurgical cares.
