再発性の巨大なエナメル上皮腫の1症例

A Casc of Recurrent Enormous Ameloblastoma

I．はじめに

　エナメル上皮腫は歯堤あるいは歯胚のエナメル器に似た組織的構造を示す上皮性腫瘍で，歯原性腫瘍の中ではよくみられる腫瘍である。本症は一般には良性腫瘍として扱われているが，手術後に再発しやすくまた再発を繰り返していると浸潤性増殖やリンパ節転移，遠隔転移をきたすことがあるとされている。

　今回われわれは30年前にエナメル上皮腫の腫瘍摘出術を受け，外観的には顔貌の変化をきたさず頭蓋底にまで達していた再発エナメル上皮腫を経験し，手術的に摘出加療しえたので報告する。

　A case of enormous ameloblastoma of the mandibular body in a 48-year-old female which recurred about 30 years after initial treatment was reported. There was no clinical manifestation but the tumor extended to the base of skull through the infratemporal fossa. Hemimandibulectomy was performed with good result.

　Ameloblastoma demonstrated a high recurrence rate. In recurrent tumor which is mostly enormous the extensive removal of the tumor with the adjacent bone was required.
