A case of nasopharyngeal angiofibroma invading the maxillary sinus and the orbit is reported.

The patient, aged 12, had been treated for sinusitis with a radical operation at the age 8 with no relief to nasal obstruction, nasal discharge and intermittent epistaxis.

At the age 12, the patient developed a right exophthalmos with a visual disturbance. The diagnosis of orbital tumor was made by an ophthalmologist who performed exenteration.

During the course of this operation the tumor was found to be originating from the maxillary sinus. The otolaryngologist discovered a reddish mass in the nasal cavity and the nasopharynx. The patient was treated with irradiation and hormonal therapy without any evident effect.

The right external carotid artery was ligated and then the tumor was removed through transpalatal and transantral approach. The tumor weighing 113 grams was completely removed and the blood loss amounted to 3, 800 ml.

The patient is now completely well with a gain of body-weight of 9 kg. in the four month period after the operation, and is free from functional and cosmetic defects.
