パッサーバン隆起とは何か—そのメカニズムと臨床的意義

SOME ASPECT OF PASSAVANT'S RIDGE

　I．はじめに

　発声時において，咽頭後壁に出現する水平隆起の報告は，1863年西独フランクフルトの外科医，Gustav Passavantによつて始められた。この報告以来，その水平隆起は彼に因んでパッサーバン隆起として名付けられ，それは，発声に必要な口腔と鼻腔との遮断（鼻咽腔閉鎖）の一助となるものとみなされてきた。この隆起が臨床上特に意味を持つのは口蓋裂患者の場合であると考えられてきた。つまり，このパッサーバン隆起が従来いわれてきた様に，口蓋裂例の鼻咽腔閉鎖を補い得るものとすればその意義はまことに大きい（第1図）。

　しかし，近年，主に英米の研究者でこの隆起の意義に異を唱えるものが現われており，かつわれわれも従来より，この隆起の鼻咽腔閉鎖に果たす役割に少なからず疑問を持つていたので，ここにわれわれの観察結果も含めて，諸家の研究成果を整理し，パッサーバン隆起とは何かを今一度振り返つて考えてみたい。

　パッサーバン隆起を論じる際に問題となる点を大別すると，

　1．水平隆起はどの様にして形成されるのか，その形成の機構は何か？

　2．隆起は何のために形成されるのか，その臨床的意義は何か？

　の2つに分ける事ができる。

　There are many questionable points concerning the significance as well as the mechanism of the Passavant's ridge. The authors add their investigative results to these questions. Today, it is almost a common agreement among various authors that the ridge is composed of contracting muscles of the upper pharynx as its contex.

　On the other hand there was a time when the concept that the clinical significance attached to the ridge was more in favor of an idea of hindering the proper closure of the nasopharynx. However, this concept is declining from popular favor, recently. The authors are also in agreement with this latter concept. Therefore, in cases of operation on the cleft palate no particular attention should be paid to this structure.
