Abstract Purpose:To report a case of adult T-cell leukemia with frosted branch retinal angiitis who was treated by vitrectomy and allogeneic hematopoietic stem cell transplantation.

Case:A 35-year-old female presented with pain and hyperemia in both eyes. During treatment for keratitis, she was found positive for antibody to human T-cell leukemia virus type 1(HTLV-1). She was diagnosed with adult T-cell leukemia and started systemic treatment with multiple chemotherapy.

Findings and Clinical Course:The right eye showed sheathing along the vascular arcades. The sheathing gradually extended to the whole retina manifesting as frosted branch angiitis. Rhegmatogenous retinal detachment developed 2 months later and was treated by vitreous surgery. Allogeneic hematopoietic stem cell transplantation was started 4 months later. The right eye showed gradual improvement of frosted branch vasculitis and of systemic manifestations.

Conclusion:A case of adult T-cell leukemia showed frosted branch retinal angiitis that improved after vitrectomy and allogeneic hematopoietic stem cell transplantation.
