網膜微小塞栓症の評価にOCTアンギオグラフィとマイクロペリメータを用いた1例

要約　目的：網膜の微小塞栓症の光干渉断層計アンギオグラフィ（OCTA）とマイクロペリメータによる所見の報告。

症例：37歳女性が，未破裂の右内頸動脈瘤に対し瘤内塞栓術を受けた。術翌日から右眼の視野中央部に微小な欠損を自覚し，術2日目に当科を紹介され受診した。

所見と経過：矯正視力は右1.2，左1.5で，右眼の中心窩耳下部に白色の病変があった。OCTAで血流欠損があり，マイクロペリメータでその部位に感度低下があった。これらの異常は4か月後に自然寛解した。

結論：眼底の微小な検出と経過観察に，OCTAとマイクロペリメータが有用であった。

Abstract　Purpose：To report a case of retinal microembolism evaluated by microperimeter and optical coherence tomography angiography（OCTA）.

Case：A 37-year-old woman was referred to us for a subtle defect in the center of her right visual field. She had received embolization for unruptured aneurysm in the right internal carotid artery 2 days before.

Findings and Clinical Course：Corrected visual acuity was 1.2 right and 1.5 left. The right fundus showed a small white spot inferior temporal to the fovea. OCTA showed defective blood flow in the spot. Microperimeter showed impaired sensitivity in the area. The findings normalized spontaneously 4 months later.

Conclusion：OCTA and microperimeter were useful in the detection and follow-up of small lesions in the fundus.
