島根県隠岐の島における網膜色素変性症の外来占有率と家族歴

Incidence and family history of outpatients with retinitis pigmentosa in Oki Island, Shimane Prefecture

要約　目的：島根県隠岐の島における網膜色素変性症（RP）の外来占有率を内地の眼科外来と比較する。また，聴き取り調査によってRPの家族歴を調べる。

対象と方法：2016年1月1日〜12月31日の期間の隠岐病院眼科外来におけるRPの病名をもつ患者の割合をデータベースを用いて調べた。同意が得られた症例において，聴き取り調査を行った。同様の検討を山陰労災病院，野島病院，智頭病院で行った。

結果：RP外来患者/総外来患者は，隠岐病院では延べ数で143/12,315名（1.16％）であった。それに対して，山陰労災病院1/8,498名（0.01％），野島病院25/16,700名（0.15％），智頭病院1/2,698名（0.04％）であり，隠岐病院以外（内地）の合計では27/27,896名（0.10％）であった。隠岐病院でのRPの外来占有率は他の施設と比較して約11.6倍であった。隠岐病院におけるRP患者33名のうち，聴き取り調査を行った32名（97.0％）おいて，RPの家族歴を有する患者は19名（59.4％）であり，うち2親等以内に家族歴を有する患者は16名（50.0％）であった。近親結婚の家族歴を有する者はなかった。

結論：施設の特性が異なることから，本検討で求められた外来占有率が各地域での患者有病率を直接表すものではないものの，隠岐のような離島では歴史的に一親族が限られた地域にとどまることが多いため，RPにかかわる遺伝子比率が高いと考えられた。

Abstract　Background：Oki archipelago is located about 50 km north of Shimane peninsula, Shimane Prefecture. The islands are populated by about 20,000 inhabitants. There is a possibility of high incidences of hereditary disorders, including retinitis pigmentosa（RP）, among inhabitants of the archipelago.

Purpose：To report the incidence and family history of outpatients with RP at Oki Hospital as compared with findings at clinics in the mainland of Shimane Prefecture.

Method：This study was made on outpatients who visited Oki Hospital, Sanin Rosai Hospital, Nojima Hospital, and Chizu Hospital during 2016.

Results：There were 143 patients who were diagnosed with RP out of a total of 12,315（1.16％）outpatients during 2016 at Oki Hospital. There were 1 out of 8,498 outpatients（0.01％）at Sanin Rosai Hospital, 25 out of 16,700 outpatients（0.15％）, and 1 out of 2,698 outpatients（0.04％）at Chizu Hospital. Incidence of RP was significantly higher at Oki Hospital than at other 3 hospitals（p＜0.01）. A total of 32 patients responded to questionnaires at Oki Hospital. There were 19 patients（59.4％）who reported the presence of relatives with RP, including 16 patients（50.0％）with relatives with RP in the second degree. There was no case with family history of consanguineous marriage.

Conclusion：Incidence of RP was significantly higher among outpatients of Oki Hospital than 3 hospitals in Shimane Prefecture in the mainland.
