Abstract Purpose:To report a case of bullous keratopathy due to contact with iris tumor treated by Descemet stripping automated endothelial keratoplasty(DSAEK).

Case:A 79-year-old female presented with visual impairment. She had a history of attending an eye clinic during childhood. She had received surgical removal of right kidney due to renal cell carcinoma, with no apparent systemic metastasis.

Findings and Clinical Course:Corrected visual acuity was 0.7 in either eye. Both eyes showed cataract. The left eye showed solitary iris tumor with iris dehiscence. Density of corneal endothelium was 2,564 cells/mm2 in the right eye and 517 cells/mm2 in the left. She showed progression of cataract with bullous keratopathy in the left eye and received cataract surgery. The removed iris tumor was diagnosed with iris cyst. Visual acuity improved to 0.6 after surgery with density of corneal endothelium of 1,441 cells/mm2.

Conclusion:DSAEK was effective for bullous keratopathy due to contact with iris cyst with the corneal endothelium.
