放射線療法が奏効した浸潤性視神経症

Leukemic and lymphomatous cell infiltrative optic neuropathy and radiation therapy

緒　言

　白血病や悪性リンパ腫の経過中に視神経への病的細胞の浸潤による視神経症をみることがある。最近2年間に白血病3例，悪性リンパ腫2例の浸潤性視神経症を経験したがいずれも放射線療法により著明な改善をみた。そこでこれらの臨床的特徴および放射線治療について検討したので報告する。

Leukemic and lymphomatous cell infiltration to the optic nerve (infiltrative optic neuropathy) occurred in 5 cases with acute leukemia and malignant lymphoma during the course of the disease.

The clinical features were characterized by severe optic nerve disturbance, disc edema, optic nerve swelling demonstrable by CT-scan, pleocy-tosis in the cerebrospinal fluid and other symptoms in the central nervous system. The infiltrative optic neuropathy responded remarkably well to radiation to the whole brain at dose levels of less than 3, 000rads. Systemic chemotherapy via rou-tine and intrathecal routes was futile.
