緒　言

　Exfoliation syndrome （以下Ex．syndromeと略す）は緑内障を伴うもの（水晶体嚢緑内障：glaucoma capsulare）と緑内障を伴わないものの2群があることは周知の事実であるが，exfoli-ative materialの本態やこの物質と緑内障の関係については未だ不明の点が多い。従来，水晶体嚢緑内障の眼圧上昇機転はexfoliative materialがtrabecular meshworkを閉塞することによるという見解が多い。われわれは先にtrabecul-ectomyより得た水晶体嚢緑内障の隅角切除片を電顕的に検討した。その結果，本緑内障の多くは既存の緑内障がexfoliative materialにより増悪または修飾されたものであると考えるのが妥当であろうと推論した^1）。しかし電顕による所見は眼組織のごく限られた部分のものであり，電顕所見だけで本緑内障の眼圧上昇機転を論ずるのは無理がある。そこで今回は本緑内障が高齢者でかつ開放隅角を有するものに多い事実から，水晶体嚢緑内障46例56眼と同年代の原発開放隅角緑内障の類似点および相違点を臨床的に比較検討し，電顕所見により得られた推論と同様な結果に達した。

We compared clinical features of 56 eyes of 46 patients with capsular glaucoma with those of 64 eyes of 38 patients with primary open-angle glaucoma of over 50 years of age. We also made a retrospective survey of glaucoma subjects from our clinical records.

Capsular glaucoma accounted for 10.4% of a total of 367 glaucoma cases of any type in our clinic during the past 5 years. Capsular glau-coma was found only in subjects of more than 47 years of age, whereas primary open-angle glaucoma manifested itself already during the thirties.
