未熟児網膜症の光凝固による治療

Photocoagulation for the Treatment of the Retinopathy of Prematurity

I．緒言

　未熟児網膜症はTerryが1942年未熟児の水晶体後方に血管を伴う組織増殖を起こす疾患の存在を最初に報告して以来，欧米において急速に増加し，1952年頃より，これが未熟児に対する酸素使用に関係あることが次第に確認され，さらにKin—seyらの広範な統計的研究の結果，酸素供給の制限によつて明らかにその発症率が激減することが認められてからいちじるしくその発生数を減じて今日に至つている。しかしその後もなお少数ながら未熟児に本症の発生が見られていることはZachariasが早くから警告しているところであり，わが国においては植村らが本症の発生がけつして稀なものでないことを1964年以来繰返し強調している。わが国においては，病院未熟児室の設備近代化が比較的近年のことに属し，本症に関する小児科医，眼科医の知識が従来ともすれば概念的なものに過ぎぬうらみがあつた。植村らの具体的経験に基づく警告によつて，ようやく本症に関する関心が高まりつつある現状とはいえ本症発生の素地はむしろ昔にくらべて高くなつているのではないかと考えられる。

Six cases of 36 premature infants developed retinopathy of prematurity in the prematurity ward in Tenri Hospital from Apr. 1966 to Oct. 1967.

In two of these cases which received relatively prolonged oxygen therapy the disease progress-ed from active Stage Ⅰ of Owens into Stage Ⅲ during the period of regular ophthalmologic ex-aminations.

In the other cases spontaneous regression occu-rred.

The lesion in both cases were on the temporal side of the fundus, and treated with photocoag-ulation.

The neovascularisation and the peripheral exu-dative lesion were successfully coagulated and mild cicatrization took place. in 2 weeks.
