ガンシクロビル硝子体注射が奏効した小児のサイトメガロウイルス網膜炎の1例

Intravitreal ganciclovir was effective for cytomegalovirus retinitis in a child

要約　目的：ガンシクロビル硝子体注射が奏効した小児のサイトメガロウイルス網膜炎の報告。症例：5歳女児。急性骨髄性白血病で血縁者間同種骨髄移植が施行された。移植後から移植片対宿主病予防のために免疫抑制剤とステロイドが投与されていた。所見：移植3週後に左眼底に網膜出血と滲出斑が出現した。前房水からサイトメガロウイルスが検出され，サイトメガロウイルス網膜炎と診断した。ガンシクロビル硝子体注射を施行した。その3週間後に網膜炎は瘢痕を残して鎮静化し，発症から7か月後も左眼の視力は1.5を維持している。結論：小児のサイトメガロウイルス網膜炎に対し，ガンシクロビル硝子体注射が奏効した。

Abstract.　Purpose：To report a case of cytomegalovirus（CMV）retinitis treated by intravitreal ganciclovir. Case：A 5-year-old girl was referred to us for check-up before start of medication for acute myelocytic leukemia diagnosed 10 weeks before. She received bone marrow transplantation followed by systemic corticosteroid and immunosuppressive drugs. Finding：The left eye showed hemorrhage and exudates 3 weeks after transplantation. CMV was detected in the aqueous leading to the diagnosed of CMV retinitis. Intravitreal injection of ganciclovir was followed by resolution of retinitis 3 weeks later. She is doing well 7 months after the onset with visual acuity of 1.5. Conclusion：Intravitreal ganciclovir was effective for CMV retinitis.
