 A 66 year-old man was admitted to our hospital because of epigastralgia. UGIS revealed the doubling of the pyloric channel which the communication between the gastric antrum and the duodenal bulb on the greater curvature. A round pit on the posterior wall of the gastric antrum near the greater curvature at the distance of the pylorus was seen endoscopically.

 Gross appearance of the resected specimen revealed double pylorus and gastroduodenal channel in the presence of a normal pylorus.

 Expressions such as double pylorus, pyloric diaphragm, antral mucosa band, and web used in the past probably described the same lesion, but we discribed the double pylorus as that formed by a nonsurgical extrapyloric connnection between the gastric antrum and the duodenal bulb. The literature on the subject is reviewed.
