In order to examine the relationship of gastric cancer to intestinal metaplasia, we found it expedient to supplement histological analysis by adapting Burstone's histochemical method for the demonstration of leucine aminopeptidase (LAP) to the gross specimen of stomachs. A similar approach, using alkaline phosphatase (ALP), has been reported. But occasionally intestinalized mucosa remained unstained with ALP method. Such metaplasia could be stained with the present LAP method. Intestinal metaplasia and duodenal mucosa were stained red. Preliminary examinations showed that areas showing a positive reaction for LAP coincided with zones of intestinal metaplasia examined histologically. Nachlas-Crawford-Seligman's histochemical method was also adaptable as the same approach.

Twenty-five specimens were examined. Areas demonstrated by LAP activity were always more extensive than those demonstrated by ALP activity. The former always included the latter. The patterns of the distribution of the metaplasia consisted of four independent types and/or their combinations. Typical each pattern of the distribution revealed to have specific enzyme-activity.

This method is quite simple to apply and never interfere with histological examination.
