外来胃集団検診の特殊性

Speciality of Gastric Mass Survey at the Out-patient Clinic

はじめに

　いままで胃の集検には多くの場合，検診用自動車を用いて特定の地域や職域に出張する集検方法がとられてきたが，昭和40年秋，有賀¹⁾は，これとならんで病院外来のごとき施設においても検診の希望者を集め，これを一つの集団と見做して検診する方法の必要性を提唱し，これを外来集検と呼ぶこととした．

　ついで昭和41年11．月広島で開かれた第4回日本胃集団検診学会秋季大会においても，外来集検がシンポジウムのテーマのの一つとして採り上げられ^2)～8)，多くの関心が寄せられるようになってきた．

　すなわち，外来集検の特性とするところは，固定された施設においておこなわれるのであるから受診者からみて責任の所在が明確であり，しかも第一次スクリーニングと，それ以降の各種精密検査および爾後の処理が一つの機関内で直結していることである．集検の場合，このような一貫した方式でおこなうことが，精検受診率も高く胃癌，とくに早期胃癌の発見率の最も良い成績が得られていることはすでに多くの人⁹⁾¹⁰⁾の指摘しているところである．

　さて，癌研病院¹⁾でも従来より検診車による集検をおこなってきたが，外来集検の必要性を感じ昭和41年6月から検診外来という窓口を新設し，病院を訪れる愁訴の少ない胃の検診希望者に対し車検診と同様に間接レ線検査を第一次スクリーニングとし，次にP_Ⅱカメラを使用するという方法での検診を開始した．その後検診外来を訪れる胃の検診希望者は次第に増加し，また車集検からの処理もすべてここでおこなうようにしてみると，検診外来だけではすでに手狭まとなり，昭和43年9月からは，これをさらに拡大充実し，精密検査終了までの業務のすべてをおこなうことのできる設備をもった検診センターへと発展した．すなわち，ここでは訪れる検診希望者にたいし外来集検をおこなうと同時に車検診の業務も加え，こと集検に関するいっさいの業務はすべてここで取扱うことにした．以来現在までに未だ僅か2年3カ月の年月しか経過していないが，昭和44年1月から12月までの1年間の成績について車集検と外来集検とを比較検討し，外来集検の特殊性について述べることとする．

　Two procedures of gastric mass screening survey, in the ambulatory unit and in the out-patient clinic, that are currently being employed by the staffs of the Cancer Institute Hospital, have been correlated regarding the groups of examinees, methods of mass survey and its results in order to find out any features peculiar to the mass survey at the out-patient clinic.

　1) Female examinees were more numerous in the out-patient group, and so were subjective complaints.

　2) No fundamental difference was seen between the two groups in the initial screening method, but the rate of people in need of, and actually having undergone thorough examination was higher in the out-patient group.

　3) Discovery rate of gastric diseases was also higher among the out-patient group.

　Since the number of people desirous of mass screening survey is increasing steadily year by year, a special organization ready to tackle with this problem in earnest should be set up together with a central institution forming the kernel of this important screening procedure. Repletion of screening setup, both in the mobile unit and in establishment, is eagerly awaited.
