内視鏡視直下のポリペクトミーと完全生検の意義

Significance of Polypectomy under Direct Vison and Thorough Biopsy

はじめに

　近年ファイバーガストロスコープ（以下FGSと略す）の開発とその応用である直視下狙撃生検法の発達によって，胃病変の診断は組織学的診断を加味してますます正確なものとなってきた．しかし，胃の隆起性病変のうち，とくに胃ポリープについては，現在の生検はまだ充分ではない．これは生検鉗子の性能上ポリープ，ことに有茎性ポリープについて生検部位の選択が困難なこと，また採取可能な粘膜の大きさと深さが期待するほど充分なものでないことから，組織学的診断は充分な正確さを保証されていない．また，最近問題にされているポリープの一部にみられる異型上皮または癌の存在の有無に関しても，生検材料からでは正しい判定を下しえない．

　著者らは従来より生検鉗子の改良を行なうとともに，とくにポリープのごとき癌との関連病変の生検には，組織をできるだけ大きく，しかも完全に採取できる方法はないものかと考えていた．一昨年以来，著者らは扁桃，鼻ポリープの切断にヒントをえて，内視鏡的にワイヤーループを用いて胃ポリープを切断し，かつ回収する方法を考案し，その方法・手技などについて発表してきた^1)-7)．この方法は良性胃ポリープの治療を主眼としたものではあるが，また完全な組織診を行なうためにも有用な方法である．本文では良性の胃ポリープと診断されたものについて，術前の生検診断と切断ポリープの全組織像とを対比した成績を中心にして，この方法が従来の生検診断の不確実さを補い，これに代る完全な生検診断法（完全生検法，仮称）として意義があることを述べたい．

　Excision of gastric polyp has been tried by means of a wire loop inserted into the gastric lumen while it has been under direct view of fibergastroscope After retrieval of the excised polyp in the course of the treatment of the injured mucosa, whole of the polyps recovered have been histologically investigated. All of 49 cases of gastric polyp (55 polyps) have been cut off with success and 31 pieces have been retrieved. On the basis of their histological studies, biopsical diagnoses before their excision have been correlated with those of the reseeted specimens.

　Biopsical procedures being employed at present are still unsatisfactory in some respect, so that overlooking of lesion and possible erroneous diagnosis must be fully taken into account before one can arrive at accurate diagnosis. Not only is it of great significance in such a lesion as gastric polyp to investigate the whole tissue specimen after its excision, but it becomes essential when one has to exclude cancer.
