高位の胃病変のX線診断

X-ray Diagnosis of Lesions in the Upper Portions of the Stomach

　「高位の胃病変」と言った場合，果してどの範囲までを指すのであろうか．それは研究目的によって異なるであろうし，また同じ診断学の分野でもX線と内視鏡では，おのおのの検査方法が全く違うので，問題となる高位としての部位が当然異なるであろう．しかし，X線診断だけでも，著者により，名称や部位の取り扱いが全く一定していない．たとえば，噴門口の水平線以上を一般に穹窿部という名称で呼ばれているが，外国では，fornix²⁾³⁾とfundus^4)～8)という言葉が用いられ，著者によって様々である．

　胃高位を示す言葉として穹窿部，fornix fundus以外に噴門部，cardiaまたはpars cardiacaなどがある．しかし，その範囲も一定しておらず「噴門を中心とする小部分」⁹⁾と，噴門を含めた広い部分とに用いられている．小部分を示すものとして，Holzknecht¹⁰⁾，Finby¹¹⁾が1インチ平方Braasch¹²⁾が2インチ平方Portis¹³⁾が3cm平方，胃癌取扱規約¹⁴⁾が2cmとしており，広範囲を示すものとして，三宅¹⁵⁾は「第1斜位で弧を描く噴門後壁が，垂直な後壁と鈍角に交叉する点以上」，松浦¹⁶⁾は「食道胃接合部下2cm以上」を噴門部とし，松永¹⁷⁾は「食道開口部の下3.5cm以上」を噴門部および穹窿部と定義している．また，沢田¹⁸⁾，藤平¹⁹⁾は「体部上1/3以上」を噴門部および胃体上部として，胃上部の病変を扱っている．

　In dealing with lesions in the upper portion of the stomach, the authors have proposed to divide the intraluminal surface of the stomach into 77 sections, representing them by the marks m^7～8_0～9 as are explained in Figs. 1 and 2. According to this division, it seems reasonable to lay down that lesions in the upper portion of the stomach belong to the sections m^7～8_0～9.

　Following this sectional method, the authors have divided the area called the upper portion of the stomach into small sections, describing the most advantageous positions of the patient for their x-ray photography, with stress laid on the cases encountered by them.

　1. In operated cases, 8.7% of advanced carcinoma and 3.3% of early one were found in the sections 111323. In other words, the rate of cancer detection in these sections was low when cancer was in its early stage.

　2. Eighty % of early gastric cancer found in the sections m^7～8_0～9 were of protruding type.

　3. For the x-ray diagnosis of lesions in the upper portion of the stomach, there are two most effective methods. The one is photographyof this part while barium meal is passing down through the cardia, and the other double contrast study in varying positions of the patient.

　4. Three cases of protruding lesions in the topmost part of the stomach were experienced (2 adenomatous polyps and 1 Ⅱa).

　5. Kissing ulcer in the sections m^7～8_0～9 rsults in the deformation of the fornix like that of the duodenal bulb.
