早期胃癌再発死亡例の病理学的検討―胃癌の胃壁深達度についての考察

Pathological Evaluation of Recurrence and Mortality in Early Gastric Cancer: Consideration on the Pace of Depth Invasion of Gastric Cancer

はじめに

　ここ10年間における本邦の胃癌早期発見の進歩はめざましく，胃癌の永久治癒という先人の悲願がようやく達せられてきた現状である．これはX線診断学を初めとし，内視鏡，さらに加えて胃生検技術の驚異的な進歩が，今日の成果をもたらしたものであることは論をまたない．

　さて，今日早期胃癌として定義されているものは，1963年第3回胃癌研究会（新潟）において「癌の浸潤が粘膜下層にとどまるもの」であった．しかし，リンパ節転移のあるものもこの枠に入れるべきか否か当然議論の対象となったが，それ以後この問題は論議されることなく，リンパ節転移を有するものも含めて早期胃癌として扱われ，現在に至っている．このようにして定義された早期胃癌もその後，年々症例が増加し，ようやく，その5年生存率の検討がせまられる時期になった．

　東大分院，林田教授は第11回日本内視鏡学会において全国の22施設から集計された早期胃癌2,364例中113例，4.7％の再発を報告した．

　また，1969年，第13回胃癌研究会（名古屋）では“早期胃癌切除後再発例の検討”の主題のもとに，再発の定義および統計，診断および外科治療，症例および病理の各項目についての報告が行なわれたが，その際，集計された早期胃癌の頻度は1,643例中，73例，4.5％であった．この数字は林田教授の集計にほぼ等しく，これらの成績から現在いわれている早期胃癌の再発死亡の頻度は4％近くとみてよかろう．

　いかなる型の早期胃癌がどのような型で再発するか，また，早期胃癌の予後を左右する因子を多少でも明らかにすることが今回の目的であるが，病理学的に詳しく調べられた早期胃癌再発死亡例の症例は少ないので自験例を中心とし，第13回胃癌研究会に報告された症例を参考として上記の問題をのべてみることにする．

　1. In Japan early gastric cancer has at present around 4 per cent of recurrence and mortality, most frequently due to liver metastasis followed by remnant cancer of the gastric stump due to its infiltration, and next by recurrent cancer without stump invasion (local or remnant stomach recurrence). Multiple cancer is least responsible for recurrence.

　2. The pace of intramucosal spread of gastric cancer has been estimated by investigating specimens of the remaining stomach at the stump.

　3. The tempo of intramucosal depth spread of depressed as well as protruding type of cancer has been estimated by retrospective studies of x-ray and endoscopic pictures of remaining cancer at the gastric stump.

　4. Of utmost importance in the prognosis of early gastric cancer, the factor of vascular invasion in relation to the pace of intramural depth spread has been discussed.
